Introduction:
The population in Hong Kong is ageing, and the number of older people with chronic conditions will continue to grow. In this relation, The Hong Kong Jockey Club Charities Trust (“The Trust”) has adopted a proactive approach to strengthen the support for senior citizens, and has earmarked over HK$340 million to carry out the five-and-a-half-year “Jockey Club Community Health Care Project” (“the Project”).
Launched in 2016, the Project adopts an innovative approach to promote preventive healthcare among the elderly and empower them in health management. It is the first city-wide elderly care support project which integrates digital health management technology, community care and professional support.
With the theme of “The Role of Digital Health in Integrated Care of Older People”, the symposium will bring together prominent scholars around the world as well as renowned not-for-profit organizations to participate and introduce the World Health Organization’s Integrated Care for Older People (ICOPE) and the application of technology for health of older people. The Hong Kong Jockey Club’s project in the area of primary care for older persons using an integrated medic-social approach following the WHO’s philosophy will also be showcased.
