Death Audits

All units in public hospitals hold regular death audits in their monthly departmental meetings, during which reviews are conducted on some of the deceased patients to examine whether the given medical treatments were appropriate and in alignment with their best interests. In this regard, the project has helped incorporate the documentation of end-of-life preferences of the deceased patients as well as the communication process into the death audit procedure. These death audits serve as an excellent learning opportunity for the doctor attendees to further appreciate the importance of transparent documentation and effective communication with patients, their family members, and other colleagues in the healthcare team.
