Jockey Club Community eHealth Care Project
Jockey Club Community eHealth Care Project (the Project) adopts an innovative approach to promote preventive healthcare among the elderly and empower them in health management. It is the first city-wide elderly care support project which integrates digital health management technology, community care and professional support.
The Project aims to empower elderly centres to identify and address the health and social needs of older people, encourage self-management of chronic conditions through telecare and nursing support, and motivate adoption of healthy lifestyle through equitable and accessible digital health technology. Health related data will be gathered and aggregated for data analytics and building up of data source on elderly health and social needs.
Project objectives
- To apply eHealth and digital health solutions to empower older people to build self-management habits
- To promote elderly centres as the first point of contact for detecting and addressing the health and social needs of the elderly for healthy ageing
- To pilot eHealth and digital health technology to improve quality of life for the elderly through innovative approach, and to collect, aggregate and analyse the health characteristics and patterns of the elderly through data analytics
